Informal Apraxia Assessment

I haven’t have too many students with apraxia, but when I do, I always feel like I need an informal tool to assess them. So what’s an SLP to do? Make one!

With this download, you’ll get a 3 page data sheet. It is black and white, so it’s easy on the ink.

The assessment itself is informal. This means you need to use your clinical judgment to decide what your student needs to work on. It does not yield standard scores. There are 4 cards on each page. Print, laminate and cut out the cards and put on a binder ring. On each card, there are 2 stimuli pictures that correspond to the data sheet. The following combinations are included:

V in isolation/words, including 4 diphthongs: 16 total
VC: 10 total
CV: 10 total
CVCV: 10 total
CVC: 10 total
CVCVC: 10 total
L-blends: 10 total
S-blends: 10 total
R-blends: 10 total
3 syllable words: 10 total
4 syllable words: 10 total
Intelligibility rating: rate words or sentences, depending on the level of the student
